Here is a patient who, over 20 years ago, we 'temporarily' bonded on some tooth colored 'composite' to help reshape her teeth. Well, after all this time, she decided it was time to update her smile and really go to a whiter shade. (the bonding we placed over 20 years ago was still on her teeth when we started!). So, she decided to go with porcelain veneers on her front upper ten teeth, and she now LOVES her new smile!
Her wishes for a more natural and beautiful smile were easy, quick and completely painless!
"This young lady had these silver mercury ('amalgam') fillings placed recently. Her previous dentist didn't give her the option for a more stronger, more 'tooth-like' restorative material. When we did a few other teeth for her at a previous appointment, she decided it was in her best interest to replace all of her 'black/silver' fillings and update them with 'bonded composites.'

She wanted to get rid of her 'black fillings' ('amalgam' which contain a mixture of silver and mercury) plus she had decay between several of these back teeth. "This is a three unit bridge made of a space-age material called "Emax (a ceramic) pressed to Zirconium."
I really like this material for this application..It's done via CAD-CAM (computer aided design-computer aided milling) for the substructure, constructed short of the margins,and then the lab waxes up the rest of it to full contour and presses the Emax to it resulting in a beautiful three unit restoration of the missing tooth, and a strengthening of the remaining adjacent teeth.
I ONLY removed the previously filling on the front tooth and a did a small conservative tooth colored filling on the gum area of the front tooth. When I talked to the lab that did this conservative bridge for us, , he said that most dentists would just cut down the entire tooth on both sides . When asked why, he says it's harder to construct what we're doing, but we retain more tooth structure for the patient. I'd rather keep more of my own tooth!"

is the hardest thing that dentists are asked to do..
Replace ONE front tooth and make it match the adjacent
one! This client fractured both of his front teeth and
the right one had to be extracted. It was replaced by
a single tooth implant and a porcelain fused to gold
crown. Even though the gum levels aren't exactly perfect,
when the time comes to do the tooth next to the implant,
the result will be ideal. Plus, he doesn't show this
area even with a full smile. Notice we also removed the
space between the front teeth. He is so happy with his
smile and he no longer has to wear a denture to replace
his front tooth. This tooth will never decay, never be
sensitive to temperature changes and will remain stable

"Here's one of our very favorite clients who requested that we remove his old silver fillings (amalgam) and replace them with stronger, more durable tooth colored restorations. We have not used amalgam for many years. The material contains about 50% mercury (plus they don't look natural!). There are alot of questions surrounding this controversial material. Many well known researchers and major dental organizations say that silver filling material (amalgam) is safe. We just don't like the idea of using it because all it does is fill the hole! It doesn't strengthen the tooth like today's adhesive dentistry, nor does it wear well over many years if alot of it is used for one tooth. Another big issue with amalgam is that it causes discoloration of the remaining tooth.
This whole procedure took less than 45 minutes and the client was so happy with the results, he immediately made another appointment to get the rest of his silver fillings replaced! These types of fillings are BONDED to the remaining tooth structure, rendering the tooth even stronger than it was before! Plus, the final results look like natural teeth! The green rubber dam seen in the picture and the blue 'sealer' used around the teeth helps keep the teeth dry while we're working, plus it eliminates sensitivity after the procedure by keeping saliva out of the teeth. It also helps prevent the client from swallowing any old filling material. It virtually separates the teeth from the client. Many clients even fall asleep during procedures like this because it's so comfortable!"
